PPP Loan Forgiveness Application Page
The SBA has approved a dramatically simplified forgiveness process for all PPP loans $150,000 and less, requiring a simple one-page attestation on Form 3508S with basic supporting documentation.

Information needed to apply:
- Loan Amount, SBA PPP Loan Number, New Horizon Bank Loan Number, and PPP Loan Disbursement Date.*
- Supporting payroll documents (i.e. 941 quarterly payroll forms for coverage period, payroll registry for coverage period, and monthly paid statements if claiming covered non-payroll expenditures such as rent, mortgage payments, utilities, software)

Once we receive and review the signed forgiveness application, we will upload your application to the SBA. The SBA will then remit the appropriate forgiveness amount to the bank, plus any interest accrued through the date of payment, not later than 90 days from receipt of a full and complete submission.
Reminder: Forgiveness applications must be submitted after funds are spent, and no later than 10 months after the last day of your 6 month covered period.
